Agreement

1. Every player is 16+ years.

2. No player is registered with another team.

3. Players 1 to 10 ALL regularly attend our church.

4. I understand this list will be sent to my Pastor and Youth Leader to verify all players from my church. My registration will not be complete until this is done.

5. I understand any request for any player changes must be in writing and approved by the local Church Pastor OR Youth Leader. All changes must be emailed/ phoned to James Mello by Wednesday 20th May.

6. I have submitted the correct first and last name for every player.
