I assume full responsibility for my child’s health, and have provided all information necessary for their care during the holiday programme. I declare that all statements on this form are true and accurate and will inform the leaders if there are any changes. I understand that it is a condition of participation to accurately complete this health record.

I have been informed of the nature of the activities and understand that there may be a small element of risk involved. I acknowledge that my child may refuse to participate in any part of the activities if they feel apprehensive about them. I agree that if they suffer injury or illness, the organisers can arrange medical treatment and emergency services if they are unable to contact me, at no expense to the SNZ Conference or Pascoe Park.

I understand that photographs may be taken and may be used for promotional purposes at the discretion of the South New Zealand Conference. I understand that if I do not wish for images of my child to be used for these purposes, I must contact the conference at snzcommunications@adventist.org.nz prior to the holiday programme and supply them with a photo of my child for identification purposes.
